*UPDATED* Keep your wellness promise for health benefits

The 2012 Wellness Options offered by the State Health Benefit Plan (SHBP) are designed to help you and your covered spouse become active participants in improving your health.  Members enrolled in one of the Wellness Plan Options (Wellness HRA, Wellness HMO, Wellness HDHP) have agreed to the terms and conditions of the 2012 Wellness Promise:

  • Complete your healthcare vendor’s online health assessment between January 1, 2012, and June 30, 2012, through www.mycigna.com  or www.myuhc.com.
  • Obtain a biometric screening that includes blood pressure, body mass index, cholesterol and glucose measures.

Note: The 2012 Wellness Promise requirements only apply to the member and covered spouse, NOT to dependent children.

To fulfill this year’s biometric screening component of the Wellness Promise, you must obtain your screening between July 1, 2011, and June 30, 2012, either at a state-sponsored screening event or by an in-network physician.

To locate a state-sponsored screening event:

  • Go to the Wellness page on the SHBP website for a list of events and links to sign up. Space is limited, so make your reservation as soon as possible.
  • Please note that no additional screening events will be scheduled. If you cannot attend any of the events listed, you will need to get your screening from an in-network physician instead.

For screening by an in-network physician:

  • Download the “physician screening form” available at www.myshbp.ga.gov, www.mycigna.com and www.myuhc.com and taking the form to your in-network physician.
  • Have your in-network physician complete the form and fax securely to your healthcare vendor, Cigna or UnitedHealthcare.
  • Make the deadline for receipt of the physician screening form by your healthcare vendor before June 30, 2012.

Once you have your screening done by an in-network physician:

  • Follow-up with your physician to discuss test results and to develop an individual health and wellness plan.

Note: Be sure to schedule your appointment as soon as possible since it may take a couple of weeks for the lab results to be processed and for your in-network physician to receive these results and complete and fax the form.  If you do not have a primary care physician (PCP), you can access an online tool at each vendor’s website to assist you in finding a physician. Developing an ongoing relationship with one physician who knows you and your medical history can lead to better overall outcomes and lower health cost.

If your biometric screening results are out-of-range, both Cigna and UnitedHealthcare have telephonic and online wellness coaching programs and resources available to help you.  The programs are available at no cost to covered members and their covered spouses.
